Cheryl Jones, RN-WCC, of Houston, TX, is a compassionate career nurse of 36 years of experience, and a dedicated mother of three. Cheryl brings to her work as a care manager a tremendous amount of experience in nursing care management for older adults, including her work in: home health care nursing, hospice care nursing, wound care nursing, and senior living clinical management oversight. Cheryl is passionate about supporting clients with aging in place, and in partnering with clients’ care teams during crisis management. She is passionate about working with seniors and their families, empowering them and supporting them in obtaining necessary resources so that they can have their wants and needs met smoothly and swiftly. Cheryl addresses the burden of care with caregivers and families, and is comfortable having emotional conversation with her clients. It is important to her that her clients’ quality of life is enhanced. Cheryl’s unique approach with clients begins with establishing what she refers to as RAPTOR (Resource Assessment and Planning Through Ongoing Review), via in-person assessments. She educates clients and their caregivers, identifies necessary resources to support clients in all areas of concern, and builds trust through consistent follow-up and ongoing communication.
